Job Description -Provides acute, chronic, preventive, curative and rehabilitative medical care to patients -Manages transitions of care, chronic disease, advanced goals of care discussion and palliative management -Refer participants to other in-house providers, including behavioral health -Document visits in the electronic medical record

Hours -Monday through Friday -Full time

Preferred 1 year post completion of clinical training Employment experience in a community health center Experience working with elderly population Medical Spanish preferred Leadership experience a plus!
